I acknowledge that East Cobb Quilters Guild, Inc. members, event participants, and/or official designees may capture images of me, either in still or moving format, at this event and that these images and my name may be published in any format.

The CONTRIBUTION to ZEFFY is OPTIONAL. To change the contribution amount, click on the drop down box and select OTHER, and set amount as desired (ZERO is a option).
